Aged care packages

Living in the comfort of your own home is an important part of maintaining your independence while you grow older.  For some however, this may become difficult without extra support.

This is where home care packages can assist – providing help with everything from housework, meals and personal care, to home modifications and assistive aids and equipment.

Living to the Max Occupational Therapy Services can provide professional consultation services to assess you and your home for the commencement of, or as part of government funded home care packages.  Depending though on the type of package you require, we will normally need to refer you for a Home Support Assessment in order to meet eligibility requirements set by the Australian government. We are very happy to help you with this process.

And, with recent changes as part of the Living Longer. Living Better aged care reforms, our Occupational Therapists can now provide you with independent consultation services to ensure you are getting the most out of your Home Care package.

This increased autonomy in how you manage your aged care package is known as Consumer Directed Care (CDC) and allows you to have complete control over how your services are provided, and by whom, regardless of your assigned home care provider.

If approved for a home care package, you will subsequently be referred to an approved Home Care provider who will assist you with the coordination of your package.

Should you require occupational therapy, this will be identified as part of your assessment.  As identified above, you may elect for your home care provider to provide and/or source this service on your behalf.  If however you have received positive feedback about our services from friends or family or should you simply prefer the independence of a third party provider, we would be happy to discuss with you.
